Service agreement

Services

We will provide you with the home care services set out in your Care Plan. Any change to the services this will be agreed and recorded in writing.

Where you wish to change the number of hours for which we provide care you agree wherever possible to give us at least 14 days' notice of the change.

We will use our best efforts to ensure that a Home Choice caregiver provides the agreed services at the agreed times. If your usual caregiver is not available for any reason we will try our best to find a suitable replacement.

Your rights

If you have any issues or complaints concerning our service we will deal with them as soon as possible. If you are not satisfied with our response then you may also have the right to complain to the Health and Disability Commissioner.

You have a number of other rights including:

The right to be treated with respect
The right to freedom from discrimination, coercion, harassment and exploitation
The right to dignity and independence
The right to services of an appropriate standard
The right to effective communication.
The right to be fully informed and to make informed choices
The right to support

Health and Safety

You must not do anything which is likely to endanger the health or safety of our caregivers.

Charges

You must pay our charges for providing caregiver services, as set out in your Care Plan.

Any overnight shifts that eventuate into a "stay awake shift"(carer is up more than
3 times during the night) the client will pay the additional charges.
Statuatory Holidays - charges occurred on these days are more than our standard charges. The client must pay these fees if they require care on that day.

Unless otherwise agreed, we will invoice you weekly. Payment is due within 7 days.

Our charges may change from time to time. We will give you at least 30 days' notice of any increase in our charges.

Reimbursement of expenses

You agree to pay any out-of-pocket expenses incurred by caregivers while providing services to you, such as costs for food or admission fees to events. In addition you must pay mileage for any travel in a caregiver's vehicle, at our standard rate. However, unless specifically agreed with you, you do not have to pay mileage costs for the caregiver travelling to and from your premises at the beginning and end of each session.

Handling your money

Our caregivers will not normally handle your money. If this is to occur a specific arrangement must be made. Our caregivers will not use your Eftpos or credit cards.

If a caregiver makes a purchase on your behalf (at your request) then the caregiver will obtain a receipt and ask you to sign it as proof of authorization.

Damage to property

Neither Home Choice nor any caregiver is liable for any breakages, spillages or other loss or damage that occurs while providing services. You should ensure that you have appropriate insurance arrangements in place.

Cleaning

All cleaning, cooking and washing products to be supplied by the client.

Privacy

During the course of providing services we will collect and record personal information (including personal health information).

We will treat all personal information in accordance with the requirements of the Privacy Act and (where it applies) the Health Information Privacy Code. This information may be used for authorized purposes. These include monitoring your care and administration. It may also be provided to any contact person (such as a family member) nominated by you.

Please contact us if you would like details of any personal information we hold. You are also entitled to request that corrections be made if you believe any of the information is inaccurate.

Changes to these terms

We can change these terms at any time by giving you at least 30 days' written notice.

Ending the service

Either of us can end the provision of services by giving the other 14 days' written notice.
